1095-C forms for Tax Year 2025 are available upon request to Truluck’s current and former employees.

Employees who wish to receive a copy of their Form 1095-C may request the form by contacting Human Resources at humanresources@trulucks.com.

Upon receipt of a request, Truluck’s will provide the Form 1095-C to the individual within 30 days, in accordance with applicable state and federal regulations.
